ECO Family Health Center is not a free clinic but does offer affordable health care.
Sliding Fee Discount qualifications may include medical and behavioral health office visits as low as $20.
Sliding Fee qualifications for dental service fees may be as low as $40.
Visit charges are determined by the level of care and services provided for each patient.
As a 330 grantee, we are expected to charge the standard rate for our area for all services. If patients do not have private insurance, Medicare or Medicaid, application for the Sliding Fee Discount is encouraged. ECO Family Health Center accepts all patients regardless of race, color, sex, religion, national origin, handicap, sexual orientation, familial relationships, or payment source.
For Sliding Fee Scale Patients
Sliding Fee Patient Policy
It is the policy of ECO Family Health Center to provide essential services regardless of the patients ability to pay. Discounts are offered depending on household income and size. A computerized sliding fee schedule is used to calculate the basic discount and is updated each year using the federal poverty guidelines. Once approved, the discount will be honored until the anniversary date of enrollment or income verification is updated. Patients must reapply annually. Required documentation includes:
Income Verification for the household (last year’s income tax, check stub, letter from employer, unemployment, etc.)
Any change of income or household size
