regardless of insurance status, ability or the inability to pay.
Health Care For You
ECO Family Health Center is not a free clinic but does offer affordable health care through our Sliding Fee Discount Program.
Sliding Fee Discount qualifications may include medical and behavioral health office visits as low as $20.
Sliding Fee qualifications for dental service fees may be as low as $40.
Visit charges are determined by the level of care and services provided for each patient.
As a 330 grantee, we are expected to charge the standard rate for our area for all services. If patients do not have private insurance, Medicare or Medicaid, application for the Sliding Fee Discount is encouraged. ECO Family Health Center accepts all patients regardless of race, color, sex, religion, national origin, handicap, sexual orientation, familial relationships, or payment source.
What is a Sliding Fee Discount Program?
A Sliding Fee Discount Program (SFDP) was created to offer healthcare to individuals who are uninsured, under insured, or have low income. The SFDP adjusts the amount a qualified patient owes for their healthcare services based on household income and size. Patients in households with lower incomes will pay less for the SAME healthcare services that are offered at undiscounted cost.
You can apply for the SFDP at any ECO Family Health Center location.
Once you are approved for the SFDP your discount is good for 1 year.
To continue to receive the discount, you must reapply each year.
How do I qualify?
To qualify for the Sliding Fee Discount Program, you must:
- Provide proof of income for all members of the family.
- Provide information on family size.
Income includes:
- wages or salary before deductions
- self-employment income
- unemployment compensation
- worker’s compensation
- veterans benefits
- food stamps
- private pensions, government employee pensions (including military retirement pay)
While the SFDP can greatly reduce the amount you owe, it does not mean that the services you receive are free. Payment is due at the time of service. If you cannot pay at the time of service, we will help you make a Payment Plan to pay out the balance of what you owe over time.
Patients who have a balance after 30 days can request a Payment Plan that will help avoid the collections process and to be able to keep getting care at ECO Family Health Center.
Please remember that ECO Family Health Center depends on you to help us to keep providing these much-needed services to our communities and patients! You can make payments by mail or in person at any of our locations.
For Sliding Fee Scale Patients
Sliding Fee Patient Policy
It is the policy of ECO Family Health Center to provide essential services regardless of the patients ability to pay. Discounts are offered depending on household income and size. A computerized sliding fee schedule is used to calculate the basic discount and is updated each year using the federal poverty guidelines. Once approved, the discount will be honored until the anniversary date of enrollment or income verification is updated. Patients must reapply annually. Required documentation includes:
Income Verification for the household (last year’s income tax, check stub, letter from employer, unemployment, etc.)
Any change of income or household size
